need to have your medical history information organized. If you have had
illness and/or treatment by a doctor you will need the doctor’s
name/address/phone, plus, dates of treatment and recovery. You can leave
the application and come back to it at any time. We can sign in with you and

your application for detail and accuracy before you submit it to the company.
